    I’m going to add some quotes here from a BAAPs registered surgeon James McDiarmid, who answered questions for us here hun in regard to whether overs or under were the best placement for women as I fear some may take that page seriously and each woman really should have her placement tailored to suit her own body type, no one placement is right for every woman.

    These responses are quoted from James’s ‘ask a surgeon’ section here:

    Definitely overs if you have enough tissue to cover the implant naturally.

    Unders are great in some patients but not great in others,

    I try to go in front of the muscle as much as possible as this produces a

    far more natural breast in terms of movement and wobbliness.

    Behind

    is an option when patients really are lacking soft tissue cover but I

    have to say I think the dual plane pocket is over-used. Evidence for

    this is the high number of patients who send me their pictures from this

    forum with a similar story. The problem I see very often is caused by a

    round high profile implant stuck up way too high behind the pectoral

    muscle creating a double-bubble appearance. Often looking at the preop

    photos it is hard to imagine why the surgeon did not choose the simpler

    option of staying in front of the muscle.

    And when asked how are unders and overs determined this was his very detailed response:

    The usual answer is that it all boils down to thickness of soft tissue

    coverage (determined by skin pinch) and to a large extent it does,

    however there are several other factors which figure in the decision.

    This

    conventional wisdom is largely based prior to the advent of form stable

    (less ripply) implants and holds very true in extremely thin ladies.

    After 20 years of medical

    practice and 10 years NHS consultant and private practice the basis

    upon which I take the decision to encourage patients to have either

    unders (usually partial unders/dual plane) versus overs has evolved

    quite far.

    I do not think that there is a correct plane for every patient indeed certain patients would have a good static (i:e photographic) result with a round or teardrop shaped implant either in front or behind the muscle.

    What I would definitely say to you

    though is that the enhanced mobility of a subglandular (over) implant

    means a more natural dynamic result. The implant will move with the

    breast gland as it is filling the natural plane that the glandular

    tissue normally occupies.

    So an ‘over’ implant will hang, wobble, feel and age more like a normal breast than an ‘under’.

    The

    enhanced tissue adherence seen with ‘unders’ means that in addition to

    being prone to move with pec major contraction the implants will tend to

    be rather fixed to the chest wall and not flop to the side when the

    patient lies down in bed or on a sun-lounger. This is both an advantage

    and disadvantage as unders will be more gravitationally stable through

    this enhanced tissue fixity. It can also make them more prone to

    development of double-bubble as the breast ages and droops and the

    implant stays up high.

    In a nutshell – for the most natural

    result go for overs if you can. Unders are great in certain patients but

    not necessary unless you are super skinny.

    Yeah i think alot of things i had been reading were so very biased with the unders and I think this helps alot to understand the benefits of overs aswell, i think its tough realizing what is actually adequate breast tissue to start with to have overs as I personally perceived mine as small xxx

    Ooooo I like this! I walked into my surgeon and said striaght of I want unders but now reading This I am going to question if overs will suit me better.

    I have allot of skin, does this count?! I have lost allot of weight, and do weights so have good pec muscles so presumed behind was a better option!

    Just over a week and now I’m questioning! Lol

    I have no choice but to have unders as i’m ”super skinny” lol (have a bum hips and thighs just thin on top), you’d actually see the outine of the implant if I had overs as have no chest fat or breast tissue to hide them, they’d look hideous on me. Athough I’m not looking forward the muscle contracting with the implant although you can see the musle contracting already as I have no upper body fat on my chest so what’s a girl to do!

    Christina, id definately say if you have alot of loose skin then overs would be better and especially if your using your muscles alot aswell doing weights, as this may effect what you can do with weights after if you had unders, generally overs are for girls who wish to fill excess skin where they have lost breast tissue for various reasons :)

    Littleme, yeah unders are probably best suited to you then, i posted this for the girlies who have been given the option like me, as at first I thought with being a 32b/30c that I didnt have enough tissue to cover the implant, I was told I could have around 400/420 unders max but if i had overs I could have 445/470 so im in a predicament as to what to go for on sizes least of all unders or overs :) xxx

    Its hard as I would class myself as super skinny tbh and I didnt think that being a 32b or 30c was adequate enough tissue to have overs but then again, what do I know haha, im going to talk about this when I go for my second meeting xx

    im really thin as well and was a 30-32A i had overs, everything thats said about them “jiggling” is true, mines bounce like natural breasts and they have the natural shape its also true about when i lay down, they dont just stay where they are, they flop :) so glad i had overs :) x

    It seems overs from research iv done are best. Less chance of problems rippling etc, less risk of not being able to get a clevage as unders tend to cause a ‘run way’ affect. My sister had overs and they move, feel and look really natural not stuck on and not moving they look so natural. Although obviously different case by case with each person x

    Yes my best friend had overs and they look amazing but seen some equally amazing unders, I think initially the overs look false but eventually drop and sit more like that natural breast, what size what’s your sister pre op hun?? X x

    Yeah I know what you mean. Although the info you put at the start the guy suggests over are more natural as can move, drop etc so not so fixed and more natural but I agree to begin I think they cn look fake till they drop. Its such a tricky one! She was a 34b and is now a 32dd and only had 265cc which is a tiny implant!! Its so annoying how much they vary!xx

    I know like one can look totally different on another so I’ve been greedy with sizes I don’t want to look stupid though, and some unders never drop, I’ve seen a few girls unhappy because they have the big gap or the implants look really stuck on and haven’t dropped even after nearly a year x x
