Medical Form

Hi girls,

My doctors are taking ages to sign the agreement form not because there’s any concerns but it being so busy.

I’ve already booked my Ba and will see surgeon Wednesday. Is there a deadline as to when it needs to be signed? My op isn’t until June so hopefully it’ll be done within next few weeks. I just don’t want it to affect future consultations with surgeon or Nurse before op.

Don’t panick Hun there’s plenty of time and mya will chase them up for it that’s what your paying the for they want you to have the procedure done they will sort it .i was just the same panicking about everything xx

Ps I was convinced my doc was not gonna sing it because I had multiple sclerosis but they did Hun xx

I had my BA in March and didn’t need to get any form signed? What’s the form for? I was never asked to get my Drs to sign anything xxx

They write to your doctor letting them no your having a procedure downe and just check the all the medical questions you answered at are correct basically that’s all I guess some people hold info back so they can have it done x

Did you have to pay for yours? I’ve been told I may be charged £15-£20 x

Yeah it’s cheeky as some doctors surgeries do charge. I was initially charged £10 but when they realised I’d hardly had any appointments in the past 2 years they refunded me the money.

I think you should be fine having it all signed off though.
